Clinical Oncology Practice Model

A clinical pharmacy practice model provides clarity and structure to services provided by a pharmacy team.

The model presented in Figure 1 is an example of a clinical practice model that can be used by a pharmacy department as a template to develop an individualized service.

  • Essential Clinical Services are provided to ALL patients who are receiving anti-cancer medications
  • Enhanced Clinical Services are provided to targeted patients in addition to essential services in patient-centred and team-based settings

This model is built on the foundation of the consensus ambulatory oncology clinical pharmacy Key Performance Indicators (REFERENCE once published - see webinar) with the following structure:

  • Clinical services are provided as part of a comprehensive pharmacy service that includes:
    • Review of orders for anti-cancer medications
    • Safe and accurate preparation of anti-cancer medications
    • Patient education, supportive care, multidisciplinary team collaboration
    • Clinical Trials and Investigational Drug Support
  • The clinical services outlined at each level must be considered in the context of the specific site.  What is an essential service for one site might be an enhanced service at another depending on several factors including resources, collaborative team expectations, etc.
  • The clinical services outlined at each level may be performed by pharmacists, pharmacy technicians, or other healthcare professionals depending on the site and the existing regulatory framework.  
  • The clinical services outlined at each level are not mutually exclusive. Certain Enhanced-level services may be integrated into the delivery of Essential services, and conversely, some Essential services may be performed when providing Enhanced care.
